Eligible applicants can submit filled in application in prescribed format along with attested copies of all necessary documents and one self addressed cover of size 12x 26 cm with postal stamps worth of Rs. 35/- to the DMHO, Vzm till 15-02-2016 by 5:00 pm.

- Cover of the application should be superscribed on right top corner as “Contract Basis Vacancy- 2016- Application for the post of CAS”.
- Age relaxation will be given as per rules.
